Student Registration

Please ask your doctor's office to use the Health Assessment Record (HAR-3). Parents need to fill out the first page, sign it, and date it. Page 2 and 3 should be filled out by the Dr.

After completion of the registration application, documents can be submitted in different ways:

1. You can upload the documents you already have directly into the application before submission
2. Documents may be dropped off at the Government Center by calling in advance at (203) 977-5312
3. You can make an appointment with your child's school
4. You can Email your documents to: ofce@stamfordct.gov
